Incident-004 "SLEEPWALK"[edit | edit source]
During a routine inspection, 004 suddenly woke up, even after being injected with 19mg of fentanyl, and managed to tear a large chunk off of the orderly's shoulder before sprinting through the open cell door and escaping his containment, luckily, Swoleson was up late playing bug eating simulatorTM and subdued him before he could attack anyone else.
